Greetings, AtB! I'm venturing a bit out of my comfort zone to host a swap here. :)

Have you ever made blackout poetry? It's easy, fun, and a great way to get a bit creative even if you're in a bit of a rut. Basically, rip a page from an unwanted book, magazine, newspaper, etc. Scan it for words that you think would make a good poem when read together, in order. Draw a box around them. For this swap, your blackout poetry will be the art on a homemade postcard. I use blank postcard backings, but you can also glue over an unused postcard that you don't like -- or even make your own if you have thick enough cardstock, though it should be no larger than 4.25" x 6". Make sure that your page is attached securely to the postcard and will not get torn while in transit -- a layer of Mod Podge usually does the trick.

On the back, write a poetry-related message to your partner. Some examples: a short poem you've written, a short poem someone else wrote (please credit the author, famous or otherwise), 3 poems you recommend, etc. Include the swap name and your username for easy rating. Address it, put a stamp on it, and send it naked to your partner (no envelope).
